0.2% prevalence inc with age
often Sx are gradual in onset, may be unilateral initially 
plantars are down going

soft monotonic voice and dysphagia
cogwheel rigidity (combination of tremor and rigidity) usually asymmetrical
poor arm swing on walking (balance is well preserved and falls should suspect different diagnosis)
postural instability is late Sx, orthostatic hypotension is common
tremor worsens with stress and tiredness
moving around in bed and difficulty turning are common early on
(Dementia is NOT a classic Sx unless lewy body late on)
anxiety and depressino are common and worrisome to pt
apathy and anxiety are common
hallucinations (visual) are common - auditory less so
sleep problems are common esp xs daytime sleepiness
bladder dysfunction, constipation and erectile dysfunction are all common (hypersexuality can also occur)
Pain, xs sweating and drooling are common
weight loss may occur
seborrhoiec dermatitis

note Sx other than classic triad may be more concerning to pt and need other Rx
