''Lentigo'' - (lentigines) 
hyperplasia of melanocytes (more - so same colour in sun) - linear in spread (spread to cell layer directly above basement membrane)
cf - Freckles (ephelis) have normal number of melanocytes but inc melanin so get darker in the sun 
or - Moles (melanocytic naevi) have nests of multi-layer melanocytes
may be .... 
simple or juvenile - from birth or childhood, not sun induced
solar or senile - develop with age due to sun exposure - may be irregular
mucosal
lentigo maligna - melanoma in situ which progresses slowly, related to sun exposure and therefore age, 2-5% evolve to LMM
lentigo maligna melanoma - as LM develops bumpy surface (vertical invasion) - treat as invasive melanoma
 
Other DD
Cutaneous horn - horn like projections of keratin usually on elevated base - may develop from - AK, SCC, seb keratosis, warts
Sebaceous hyperplasia - multiple, greasy, may have central depression
Seborrhoeic Dermatitis -redness scaling, usually itch, common, occurs where sebaceous glands are active, may co-exist with AK
Intradermal naevus -pigmentation varies but rarely translucent like BCC. Hairs from lesion suggest naevus
Atypical naevus - larger, variegated, asymmetric, irregular outline
Compound naevus - combined junctional and dermal - dark, dome, smooth
Seborrhoeic keratosis (BCP) - stuck on, elderly, may itch elderly, colour varies, may have horn
Malignant Melanoma - vary ..... flat, nodular, ulcerative, pigmented or not
Discoid lupus - sun exposed areas, well demarcated, erythematous, hyperkeratotic plaques with follicular plugging. leaves atrophic scar
Discoid eczema - clearly demarcated, itchy, scaly, erthematous plaques - coalescing vesicles and papules - maybe exudative / crusty, heal with scarring
