GCS <13 at 1st assess or <15 2hrs post injury
Fracture - open / depressed skull or suspect base of skull (panda eyes, CSF leak nose or ears, haemotympanum, battle sign (bruise over mastoid))
Fit post injury, focal deficit
vomiting >1 episode
amnesia / LOC with coagulopathy

Under 8hrs if amnesia / LOC
65+ yrs
dangerous mechanism (>1m or involve motor vehicle)
amnesia >30mins pre event
any Hx bleeding / clotting disorder - or on warfarin

Under 16yrs
witnessed LOC or amnesia >5mins,
abN drowsiness
vomiting 3 or more episodes
suspect NAI - bruise, laceration <1
GCS <14 (>1) or <15 (<1)
plus all above (1 and 8 hrs)
