In the younger women this is related to varus of the 1st MT due to genes (incomplete penetration of autosomal dominant gene) and exacerbated by poor footwear. In the elderly it is related to weak muscles and RA. Conservative Mx can help pain but doesn't stop progression - includes physio and good footwear/ padding. There are a number of surgical procedures which are appropriate after conservative measures (including podiatry referral) have failed and Sx cause significant functional impairment.
